GlobalOptions Employees & Principals/Owners of Vendor Network

Vendor Network Organizations

Employees and the Principals or Owners of Vendor Network Organizations will be asked to provide the following:

• Legal name
• Address
• Social Security number
• Date of birth
• Professional license information

GlobalOptions Employees are required to provide this additional information:

• Driver's license
• Auto insurance card or certificate
• Resume in standard GlobalOptions format 

Vendor Network Organizations must provide information for its principal/owner, (see list to left) in addition to entity information as follows:

• Legal name
• Marketing name (DBA)
• Type of legal organization (corporation, LLC, sole proprietorship, etc.
• Employer Identification Number (EIN)
• Contact information including email
• Address
• Web site URL
• Professional license information
• General liability and workers' compensation insurance certificates

• Vendor Agreement
• Vendor Investigative Operations 
• Confidentiality Agreement
• Guarantee Statement
• Hold Harmless Agreement
