GlobalOptions Employees & Principals/Owners of Vendor Network

Vendor Network Organizations

Employees and the Principals or Owners of Vendor Network Organizations will be asked to provide the following:

Legal name
Address
Social Security number
Date of birth
Professional license information

GlobalOptions Employees are required to provide this additional information:

Driver's license
Auto insurance card or certificate
Resume in standard GlobalOptions format

Vendor Network Organizations must provide information for its principal/owner, (see list to left) in addition to entity information as follows:

Legal name
Marketing name (DBA)
Type of legal organization (corporation, LLC, sole proprietorship, etc.
Employer Identification Number (EIN)
Contact information including email
Address
Web site URL
Professional license information
General liability and workers' compensation insurance certificates

Vendor Investigative Operations
Confidentiality Agreement
Guarantee Statement
Hold Harmless Agreement
